We are excited to share that our Board has approved $6,235,000 in total grants this year to advance health equity in our service region.
Communities continue to face the lasting effects of public health crises, economic strain, and a challenging social and political environment threatening safety and well-being. In response, we are deepening our commitment to investments that strengthen local networks of care, amplify community voices in leadership, and improve the conditions that influence health.
Flexible, unrestricted support is critical to fuel community-led solutions, especially in this moment. Communities know best what they need, and our role is to help those solutions take hold and grow.
Our Multi-Year Partnerships: Building for the Long Term
In 2024, we launched five-year partnerships with 32 organizations, representing a total of $22.25 million in unrestricted support. This year marks Year 2, with $4,450,000 awarded.
Systemic change takes time. With these long-term commitments, partners receive stable, flexible funding to stay responsive to challenges, strengthen their teams and infrastructure, and address the root causes of health inequities through local, community-driven solutions. We support efforts that meet immediate health needs and long-term strategies, such as civic engagement, policy advocacy, and community leadership development. This can help shift the everyday conditions that shape community health, from mental health and housing to food access and affordability.
Our 2025 Annual Grants: Supporting Community Needs
We are also awarding $1,785,000 in one-year grants to 37 organizations addressing health needs and strengthening local leadership. These grants keep us responsive and learning from partners about emerging challenges and opportunities in our region.
Our Commitment to Long-Term Change
Our communities are facing complex challenges, highlighting the need for sustained, systems-focused work. Achieving health equity goes beyond expanding access to care—it means reimagining health, housing, food and other systems that shape daily life. This work calls for flexible, long-term support that addresses root causes, not just symptoms. We are moving toward a grantmaking approach that prioritizes efforts to strengthen community leadership, build organizational capacity, and transform the systems that impact daily life, creating healthier, more equitable communities for generations to come.
We look forward to working alongside our partners, our peers and other stakeholders across our region to advance this long-term vision for change.
