Our grantmaking approach is fueled by a comprehensive analysis of local health data, evidence-based research/practices and community input. This information has allowed us to expand how we think about health equity and how we can work with our communities to bring about effective solutions. Our desire is to make this critical information accessible to our partners so they can have a more complete picture of the communities we serve.
We are also energized by and believe in the power of the stories our communities have. We invite all to meet our grantee partners, community residents and work that helps to achieve our mission and vision in our service region.
In 2018 we worked with Sinai Urban Health Institute to measure and rank the social determinants of health in the communities we serve. Download the report here.
